Please see the attached information regarding our upcoming sports physical day. SIH will be providing sports physicals at Carterville High School to athletes, dancers, and cheerleaders on May 22, 2026 from 8am-12pm. These physicals are for Carterville High School students only. Appointments are required. Please call the office at 618-985-2940 to schedule an appointment. Physical forms can be found at IHSA.org or picked up in the CHS office. Parent/guardian must complete the health history sections of the form in advance of the physical.
ALL INCOMING FRESHMAN are required to get a Certificate of Child Health Examination per the State of Illinois. A sports physical DOES NOT meet the requirements for a state physical.
A $10 donation is suggested. Please make checks payable to CHS Athletics. All proceeds are given back to participating school athletic departments.
