**If you have any concerns about your child's vision or hearing please contact your school nurse.

All students must have a Medication Authorization form signed by parent and health care provider before any medication can be given at school. This includes Over the Counter medications such as Tylenol and Ibuprofen.

DENTAL EXAMS

Proof of a dental exam is required for students currently in Kindergarten, 2nd grade, 6th grade, and 9th grade and is due by May 15 of the current school year.

Dental exams must be completed by a licensed dentist and must be dated within 18 moths prior to the May 15th deadline. Waiver forms may be submitted to the nurse if you are unable to find a dentist that accepts your insurance or it is an undue burden.
